The medium-dose range refers to the ICS component as per the NHLBI NAEPP EPR3 Guidelines for the Diagnosis and Management of Asthma (2007). Maintenance ICS ranges are different for age 12+ yrs vs. younger asthmatics, but medium-dose ranges are easily satisfied with the included preparations, which include formoterol in combination with budesonide 160 mcg/actuation (0.16 mg/actuation) and formoterol in combination with mometasone 200 mcg/actuation (0.2 mg/actuation).),(Exclusion Criteria: Excludes withdrawn preparations and retired codes (although they may still be available from pharmacies until supplies are exhausted); excludes low-dose oral ICS-formoterol preparations that are less likely to be used to treat asthmatics age 4+ in the medium-dose ranges as per the NHLBI NAEPP EPR3 Guidelines for the Diagnosis and Management of Asthma (2007).)
